Default Tranny Problems...

I've had my 1992 Ford Explorer for approximately 5 years now. I did notice there was something wrong with the way it shifted. In the morning, I would fire it up and put it in reverse to get out of ther driveway. It would take a minute or two before it engaged....Well, this morning when I began my journey to work, the rear end shuddered when I accelerated and it wouldn't budge no matter how hard I pushed the accelerator. After returning home, it would not go in reverse, only forward. I drained the pan, changed the filter, and refilled it with tranny fluid. Still, no reverse. I would appreciate any feedback as to what the problem and or possible solution may be.
