Say - where is your ICP sensor located? If it is behind the turbo, then your 04 has an 03 engine. If it is on the passenger valve cover, then it is the 04 engine.
Also, have you made sure the MAP sensor hose doesn't have any leaks?
Is the port for that hose (on the intake manifold) clear and not plugged up (get a thin wire to "rod it out").
If you have the late 04 truck, then your PCM uses the EBP sensor output. You might pull the EBP sensor and see if it is clogged up. That said, you should be able to run just fine with the EBP sensor unplugged as a trial.

Took it for a drive without icp plugged in, problems persisted unchanged unfortunately. Although while driving it I heard a weird click or something that sounded like something got unstuck, and then the truck ran 'o.k' for the rest of the drive home. I noticed the turbo almost sounded like it was barely spooling if at all sometimes, but I couldn't tell for sure. Once fuel is ruled out (fingers crossed?) tomorrow would it be safe to say the turbo vanes could be a possibility? Not sure how hard it is to get to the turbo and pull it to clean it but I reckon I could do it over this entire week at least.

I would make a run with the EBP sensor unplugged for another test before removing the turbo. The BP plays an important role in turbo operation. That said, if the PCM knows that the sensor isn't working, it will infer a value for it from other sensors. It should run reasonably well that way.
Pulling and cleaning the turbo isn't a fun job, but lots of people do it. Lots of internet help and youtube videos on it.

So today I had some time to jump back onto the problem. I pulled the ebp and am replacing it and the piping for it. Unplugging the ebp changed nothing unfortunately.
Anyways, I did end up resolving the issue. Of course it was the easiest thing to think of and what I had in the back of my mind the whole time -- the diesel I had in my tank was bad. After I had ran the tank nearly dry today I dumped another 6 gallons of fresh diesel in there mixed with injector cleaner. The problem vanished and now the truck runs/starts great (much better than I've ever had it run before).
The turbo cha cha cha is happening a little more often now, is that normal? It feels like I am building more boost than I should be (do not have a gauge for it).
