Default Clutching problem

I did a search and did not find this specific issue.

A have a 1995 F250 Powerstroke, standard shift.
When ever shifting You need to bottom the clutch pedal to the floor to get good engagement and easy up or down shifting.
If you in Neutral, Its very hard to get into reverse. Awlays get taht grinding sound unless you really bottom the pedal out.
Its as if the clutch is not fully disengaging unless its way past the normal clutch travel.
We pull horse trailers with this and ther is no slippage so I ruled out a worn clutch plate.
I've looked and can find no adjustment that would alter the pedal stroke.

Is there something i'm missing?

Thank you
