rear differential with limited slip

rear differential with limited slip
Have a 1994 4x4 f150 with a limited slip rear differential. Shifting from reverse to drive is a significant bang in the rear end, shifting to reverse from drive is a really loud bang.
I am guessing the ring and pinion is probally on the wore out side and needs reset or replaced??? I am not familiar with the limited slip inner workings is there anything that could be causing this banging connected to the limited slip??
there is 191,000 on the truck.
Also this truck has factory sticker of 15 in. wheels inside the door. the truck has 16 in wheels on it. Did ford ever do that kind of switch at the factory or dealer when new back then??
